Effective Date: December 16, 2010.
1. Information Collection & Use
a. Personally and Non-Personally Identifiable Information
The Sites collect both Personally Identifiable Information and Non-Personally Identifiable Information from its visitors. “Personally Identifiable Information” is information that can personally identify you, such as your full name, phone number, email address and your postal address. “Non-Personally Identifiable Information” is information that may be personal to you, such as your demographic information or favorite web page, but which cannot be used to identify you.
We also may employ a software technology that help us better manage content on the Sites by informing us what content is effective (“Web Beacons”). We may tie the information gathered through such technology to our users’ Personally Identifiable Information.
As is true of most sites, we automatically gather certain information (such as internet protocol (“IP”) addresses, browser type, longitude and latitude, internet service provider (“ISP”), referring/exit pages, operating system, date/time stamp, and clickstream data) and store it in log files. We use this information, which does not identify individual users, to analyze trends, to administer the Sites, to track users’ movements around the Sites and to gather demographic information about our user base as a whole. We may also link this data to Personally Identifiable Information.
c. Information Collected
For each visitor to the Sites, our Web server automatically recognizes the visitor’s domain name and email address (where possible). We may also collect the domain names and any email addresses of visitors to the Sites, aggregate information regarding the web pages visitors’ access or visit, and user-specific information regarding the web pages visitors access or visit.
We may also collect the email addresses of those who communicate with us via email, and any other information volunteered by a visitor, for example, in site registrations, surveys or other requests for online feedback, which would include information such as name and address, telephone number, and fax number.
WE DO NOT ASK FOR SOCIAL SECURITY NUMBERS and urge you to ignore any emails, telephone calls, written solicitations and all other forms of communication that any unscrupulous person(s) may use to falsely represent the name of HSO when asking you for your social security number.
d. Use of Information
The information we collect is generally used to improve the content of the Sites, provide technical support or account maintenance, customize the content and/or layout of our pages for each individual visitor, notify visitors about updates to the Sites, and to contact visitors for marketing purposes.
We may, from time to time, send you emails regarding our services. In addition we may occasionally send you direct mail about products and services that we feel may be of interest to you. Only HSO (or agents working on behalf of HSO and under confidentiality agreements) will send you these direct mailings and only if you did not opt-out of these offers. If you no longer wish to receive our promotional or informational communications, you may opt-out of receiving them by following the instructions included in each communication or by emailing us at firstname.lastname@example.org or write to us at HSO, c/o Dr. Ruth Frank, Pennswood Village, 1382 Newtown-Langhorne Road, Newtown, PA 18940. Additionally, at any time, you can edit your account information to no longer receive such offers and mailings.
e. Disclosure of Information.
We may disclose your Personally Identifiable Information to our third party service providers, including companies we retain as consultants to conduct research on our behalf. We will encourage our service partners to adopt and post privacy policies. The use of your Personally Identifiable Information (such as data required for a login access to some sites) by our service partners is governed by the privacy policies of those service partners and is not subject to our control. We do not share your Personally Identifiable Information with any third parties for marketing purposes or partner with or have special relationships with any third party ad-server companies; however, we reserve the right to use or disclose Non-Personally Identifiable Information in any manner.
We may disclose Personally Identifiable Information to third parties without your consent in the following circumstances:
i. if we believe there is a serious and imminent threat to the life, health or safety of yourself or another person (for example, if we learn that a person is suffering abuse or has made statements of an intent to commit self-harm or harm another);
ii. to investigate or report on activity which we believe on reasonable grounds to be unlawful;
iii. if disclosure is required or authorized by law (for example, in response to a subpoena or where a regulatory authority has the power to request the provision of certain records or information);
iv. if disclosure is reasonably necessary to enable an enforcement body to perform its functions, for example:
a) the prevention, detection, investigation, prosecution or punishment of criminal offenses; and
b) the preparation for, or conduct of, proceedings before any court or tribunal, or implementation of the orders of a court or tribunal.
v. If we deem such disclosure necessary, in our sole discretion, to protect our legal and legitimate business interests.
We may provide aggregate information regarding the web pages visitors to third parties. For example, we might inform third parties regarding the number of users of the Sites and the activities they conduct while on the Sites. We might also inform a pharmaceutical company (that may or may not be an advertiser on the Sites) that "30% of our users live east of the Mississippi" or that "25% of our users have tried alternative medicine." Depending on the circumstances, we may or may not charge third parties for this aggregate information. We require parties with whom we share aggregate information to agree that they will not attempt to make this information personally identifiable, such as by combining it with other databases.
In the event we are acquired by or merged with another organization, all information, including Personally Identifiable Information collected by the Sites, may be shared or transferred to the new organization. We will notify you of such transfer by posting notice on the Sites if the intended use of the Personally Identifiable Information will be contrary to this policy; in the event we fail to notify you, your Personally Identifiable Information will be transferred subject to this policy.
2. Updating and Correcting Information
If your Personally Identifiable Information changes, or if you no longer desire our service, you may correct, update, or delete such information by making the change on the account information portion of the Sites or by emailing our Customer Support at email@example.com or by postal mail at HSO, c/o Dr. Ruth Frank, Pennswood Village, 1382 Newtown-Langhorne Road, Newtown, PA 18940. Please include your name and email address when you contact us.
You may ask to have the information on your account deleted or removed; however, because we keep track of past transactions, you cannot delete information associated with past transactions on the Sites. In addition, it may be impossible for us to completely delete your information without retaining some residual information because of backups.
3. Personally Identifiable Information in Public Areas
We will attempt to implement security measures in our physical facilities to protect against the loss, misuse or alteration of information that we have collected from you at the Sites. While we use reasonable efforts to safeguard the security of your personal information, any transmissions made on or through the Internet are vulnerable to attack and cannot be guaranteed to be secure. In addition, submissions made via email are not protected by SSL technology and are vulnerable to interception during transmission.
No data transmission over the Internet or any wireless network can be guaranteed to be 100% secure. We cannot guarantee that your data will be completely safe or secure because no data transmission or electronic storage of data over the Internet, or by any other means, is guaranteed to be 100% secure, even if you follow all the recommended steps for security. Therefore, while we strive to protect your Personally Identifiable Information, you acknowledge that: (a) there are security and privacy limitations on the Internet which are beyond our control; (b) the security, integrity, and privacy of any and all information and data exchanged between you and us through the Sites cannot be guaranteed; and (c) any such information and data may be viewed or tampered with in transit by a third party.
